Checking in monthly has never been easier!  Regularly checking in allows us to track your acne clearing progress so we can accurately tweak your routine to keep ahead of new acne formation while we heal past breakouts and calm inflammation.

Please take the following photos with your phone or webcam and submit by uploading at the bottom of the form below:

Remember - not too far, not too close, good lighting, and clear!  No makeup please!

1.   head-on view of the entire right side of your face

2.  head-on view of the entire left side of your face

        (full profile, not angled from the front of your face)

3.  head-on view of just your chin

4.  close-up but clear view of your nose from your brow to below your mouth

5.  head-on view of just your forehead

**Please do not send the same single front view photo for chin, nose and forhead, as this necessitates time taken to crop and adjust for size.  Please also make sure to upload photos in original sizes since necessary cropping can make photos unusable when enlarged afterwards.
